Ohio Licensed Care Manager

A company is looking for a Mom and Baby Care Manager.

Key Responsibilities

Engage members and their support systems through strength-based assessments and a trauma-informed care approach

Develop and update individualized care plans in collaboration with the inter-disciplinary care team

Facilitate communication and coordination among members, providers, and community resources to achieve care goals

Required Qualifications, Training, and Education

Nursing degree from an accredited nursing program is required

Advanced degree associated with clinical licensure is preferred

A minimum of three (3) years of experience in nursing is required

Three (3) years Medicaid and / or Medicare managed care experience is preferred

Current unrestricted clinical license in state of practice as a Registered Nurse is required
