Key Responsibilities :

Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.

Implement evidence-based interventions to achieve desired patient outcomes, including medication administration, wound care, and education.

Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services.

Communicate effectively with patients, families, and healthcare teams to ensure that all stakeholders are informed and engaged in the care process.

Requirements :

Bachelor's degree in nursing from an accredited program.

Current licensure as a registered nurse in the state of practice.

At least 2 years of experience in ambulatory care nursing, preferably in a similar setting.

Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Demonstrated proficiency in electronic medical records (EMRs) and other relevant technology systems.
