Registered Nurse Case Manager

A company is looking for a Case Manager / RN.

Key Responsibilities

Manage individual caseloads and develop case management plans incorporating communication with healthcare providers, payers, and patients

Assess patient needs, set goals, and document actions taken to achieve those goals

Maintain case documentation and billing in the computer system while ensuring quality patient care and compliance with company policies

Required Qualifications

Current active and unrestricted Registered Nurse license in the state of practice

Bachelor's degree in a health-related field preferred

7 years of varied clinical experience preferred

URAC-recognized certification in Case Management within 3 years of hire

Familiarity with community resources and funding sources for patient support
