Job descriptionWork closely with a diverse team, including case workers, chaplains, and hospitalists. Facilitate communication and coordination among team members to ensure holistic patient care. Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes. Provide disease and treatment education to patients and their families. Offer emotional support, counseling, and guidance during difficult times. Assist patients in making informed decisions about their care. This position operates during regular business hours, Monday to Friday, from 8 : 00 am to 5 : 00 pm. Compensation is calculated based on your experience and qualifications. Advanced Practice Certification : You must be a certified registered nurse practitioner (CRNP) with a focus on palliative care. State Licensure : Hold a valid Alabama state license as an advanced practice nurse. Experience : Prior experience in palliative care or a related field is preferred. Compassion and Empathy : Demonstrate a genuine commitment to providing compassionate care to patients and their families. Job Description
As a Palliative Care Nurse Practitioner, you will provide compassionate care to patients facing chronic, complex, and life-threatening conditions. Your role involves collaborating with a multi-disciplinary team to enhance the quality of life for patients and their families. Here are the key responsibilities :
Patient Assessment and Management :
- Conduct thorough assessments of patients’ physical, emotional, and psychosocial needs.
- Develop individualized care plans to address symptom management, pain control, and overall well-being.
- Prescribe medications and treatments in accordance with state regulations.
- Order routine and diagnostic studies as needed.
