Job description
To support patients with advanced illness, the full-time New York Licensed Nurse Practitioner will conduct home and telehealth assessments, manage symptom treatment, and collaborate with an interdisciplinary team to facilitate high-quality palliative care. Key responsibilities Conduct in-home and telehealth assessments, formulating diagnoses and delivering care aligned with patient goals Prescribe medications, perform medication reconciliation, and facilitate family meetings regarding care plans Educate patients and families on disease trajectories and coordinate care with healthcare providers Required qualifications Master's degree in nursing or an approved supplemental education program for Nurse Practitioners Current New York State License as a registered nurse and certification as a Nurse Practitioner National Board Certification in Advanced Practice Nursing (for NPs hired after June 1, 2019) 3+ years of experience in palliative care or geriatrics DEA certificate and Basic Life Support (BLS) certification required
