Overview

The Speech Language Pathologist provides in-home speech therapy services to patients, delivering high-quality care in collaboration with a multidisciplinary team. This role focuses on improving communication and swallowing abilities, while promoting independence and quality of life for patients.

Key Responsibilities

Provide speech therapy services to patients in their homes

Assess, plan, and implement individualized therapy programs to improve speech, language, cognitive-communication, and swallowing disorders

Collaborate with the care team to develop and adjust patient care plans based on progress and changing needs

Educate patients, families, and caregivers on treatment techniques and at-home exercises

Accurately document evaluations, progress notes, and discharge summaries in electronic medical records

Maintain compliance with professional standards, policies, and regulatory guidelines

Participate in team meetings, case conferences, and professional development activities

Schedule

Part-time (approximately 20 hours per week), Monday through Friday, 8 AM to 5 PM

Approximately 15 units per week

No weekends, holidays, or on-call requirements

Qualifications Required

Master's degree or higher in Speech Language Pathology

Minimum 1 year of experience in a healthcare setting

Experience with Electronic Medical Records (EMR), including TruChart EHR or similar systems

Valid Speech Therapist license in Michigan (MI)

Valid Regular Driver License (DL)
