Job Description :
House of Hope, a nonprofit organization, is looking for a detailed-oriented and well-versed Case Manager to join our team! As a Case Manager your position is essential in coordinating the care and health management of the clients.
Experience, Knowledge, and Skills :
- Bachelor’s Degree Required.
- Two (2) years’ experience working in addictions / mental health.
- Ability to work with a diverse population.
- Effective communication skills
- Must be an independent thinker.
- Computer Literate
Duties and Responsibilities :
- Collaborate with each client to develop their individual service plan.
- Network with agency and outside service providers, ensuring client access to appropriate resources.
- Orient clients upon phasing up, explaining all job search policies and procedures.
- Complete required training and chart audits to ensure compliance and quality of services provided.
- Communicate effectively with all treating professionals regarding clients.
- Additional tasks and duties as assigned.
