Job Summary :
We are seeking a dedicated and compassionate Bilingual in Spanish Peer Support Specialist / Case Manager to join our team in Oregon. The ideal candidate will leverage their lived experience and professional training to support individuals facing mental health challenges, fostering empowerment and self-advocacy. In addition to providing peer support, the candidate will also perform case management duties, and offer support groups to help clients navigate services and resources effectively.
Key Responsibilities :
- Provide peer support to clients, using personal experiences and recovery strategies to assist them in their journey towards mental wellness.
- Develop and implement individualized support plans, incorporating input from clients to address their unique needs and goals.
- Identify and connect clients with appropriate community resources, services, and supports, ensuring they have access to necessary care.
- Conduct assessments and evaluations to inform case management services and monitor client progress.
- Collaborate with multidisciplinary teams, including mental health professionals, to coordinate comprehensive care.
- Facilitate supportive group sessions and workshops as needed, fostering a sense of community among clients.
- Maintain accurate and confidential records of client interactions, progress notes, and case management activities.
- Advocate for clients’ rights and needs in various settings, including healthcare, education, and social services.
Qualifications :
Current certification as a Peer Support Specialist (PSS), Peer Wellness Specialist (PWS), Youth Specialist (YS), or Family Support Specialist (FSS) in the state of Oregon.Certified as a Qualified Mental Health Associate (QMHA) or Qualified Mental Health Professional (QMHP).Prior experience in peer support, case management, support groups, or mental health services preferred.Strong interpersonal skills and a passion for helping others achieve mental health and wellness.Familiarity with local mental health resources and community services in Oregon.Excellent communication and organizational skills.Compensation & Benefits :
