AmeriCorps Outreach Specialist - School Based

In support of the mission of Nationwide Children’s Hospital, the AmeriCorps Outreach Specialist will collaborate with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with unmet social needs.

Responsibilities

Duties Include :

  • Assisting patients and families with unmet social needs and improves patient / family’s access to community resources.
  • In collaboration with clinical staff, linking patients / families to essential support services including housing, nutrition assistance, utilities, transportation, and other needs as identified by the patient / family and clinic team.
  • Assisting in conducting follow-up calls to patients and families to confirm linkage with identified community resources and / or services, assesses potential barriers, and aids in navigating resources and services.
  • Providing appropriate documentation of patient and family interactions in the electronic medical record in a timely manner.
  • Developing an expertise in making referrals to housing resources, food pantries, utility companies, government agencies, treatment facilities and other internal and external resources, including understanding programs’ eligibility criteria, intake process, etc.
  • Collecting and report data on program activities.
  • Complies with all AmeriCorps program guidelines by maintaining service hours, training hours, and service projects as outlined in the AmeriCorps Service Agreement.
  • Interact in a positive, professional and cooperative manner with families, and other center staff; refer appropriate questions or concerns to AmeriCorps Program Coordinator.
  • Attend all required training courses and staff meetings and participate in AmeriCorps service projects.

Qualifications

Minimum Qualifications :

  • High School Diploma or equivalent
  • At least 18 years of age
  • Ability to pass criminal background check and drug screen
  • Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.
  • Ability to work as a part of a team, with diverse populations, (i.e. racial / ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.
  • Strong attention to detail and organizational abilities required. Sound decision making skills, with the ability to maintain highly confidential information.
  • Moderate computer skills required (familiarity with Microsoft Word and Excel, required).
  • Ability to prioritize tasks, problem solve, and work autonomously.
  • Ability to work flexible hours, which may include some evenings and weekends.
