Responsibilities
Under the direction of the Manager of Social Work, performs a wide variety of duties in the hospital setting providing behavioral health and complex care management services to Southcoast Health patients across the hospital system. Duties include developing, evaluating, facilitating, and monitoring all components of assigned guardianship / conservatorship caseloads including individual and family sessions. Responsible for initiating, establishing, and maintaining client charts and electronic records who have guardians / conservators. Serves as a consultant and resource to hospital staff in the management and treatment of guardianships and conservatorships. Collaborates and coordinates with complex / chronic care management and longitudinal community support services. Provides functional or technical supervision to LCSWs, Community Health Workers, as well as peer recovery coaches.
Qualifications
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation : Pay rate will be determined based on level of experience.
Pay Range
USD $74,256.00 - USD $104,499.20 / Yr.
