Discharge Planner

Overview

Functions as part of the multi-disciplinary team in planning appropriate treatment and continuing care plan for patient.

Responsibilities

Plays an integral part in discharge planning of the patient and assists the patient/family in choosing/utilizing appropriate services. Responds to referrals.

Qualifications

Fluent in medical terminology. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. BLS Certification.
