Pennsylvania Licensed Care Connector

A company is looking for a Care Connector - Special Needs Unit.

Key Responsibilities

Serve as the primary contact for the Special Needs Unit, assisting in resolving patient / member concerns and maintaining satisfaction

Manage crisis situations and provide assistance with social determinants of health

Collect and record data on special needs organizations and prepare communications for patients and members

Required Qualifications

Bachelor's degree in Social Sciences or a related field is required

Minimum of 1 year of related work experience is required

Experience in crisis management and patient advocacy is preferred

Familiarity with community resources and healthcare services is beneficial

Ability to navigate multiple computer programs while assisting patients / members / providers
