Position Summary

The Clinical Nursing Consultant II (Transplant Liver Coordinator) will typically develop and implement nursing educational programs for nursing service personnel in a clinical service. The Clinical Nurse Consultant II is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Clinical Nurse Consultant II provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice. The CNCII strives to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice Relationship Based Care and standards of nursing excellence into clinical practice. Through therapeutic relationships with patients, families, self and colleagues, the Clinical Nurse Consultant II participates in monitoring, developing, and evaluating nursing practice while fulfilling unit and organizational goals.

Duties & Responsibilities

• Evaluates patients at the appropriate UI Health site of care.

• Create and maintain transparent patient care workflow to ensure patient safety and support team dynamics.

• Provide emotional support to patients and their families, addressing any concerns or anxieties they may have throughout the transplant process.

• Coordinates care for transplant recipients from referral and /or until waitlisted, all potential recipients with living donors and all living donor patients.

• Ensures that all patient coordination is documented and comply with UNOS, CMS transplant program guidelines and UI Health policies/procedures.

• In collaboration with the provider, orders all testing for evaluation as needed and reviews results with the provider in a timely manner following UNOS and CMS guidelines.

• Develops, coordinates, and communicates the patient's plan of care based on ongoing assessment of the patient and input from the patient's family.

• Uses clinical expertise to collaborate with multi-disciplinary health care teams to improve patient outcomes.

• Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.

• Assesses, engages, and obtains clinical information from multi-disciplinary team, patient, and/or family to develop an appropriate patient educational plan.

• Recognizes symptoms, establishes priorities, and responds to patients' changing needs throughout the transplant care continuum.

• Ensure that the patient is added to the UNOS waiting list and the required listing letters are sent promptly per UNOS requirements.

• Coordinates and schedules all living donor transplant surgeries as assigned.

• Attends and participates in transplant QAPI meetings, listing meetings, department meetings, multidisciplinary planning meetings and other meetings as needed.

• Provides cross coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned.

• Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides, support and feedback

• Shows commitment to UI Health's values through teamwork and effective communication to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe, and therapeutic environment.

• Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide “quality" patient care as measured by established nursing quality standards and other governing/accrediting health care bodies.

• Recognizes, supports, and participates in Shared Leadership as the selected method of nursing governance through volunteering for one area of shared governance.

•Participates in organizational, departmental, and shared leadership activities; supports department and unit goals in daily behavior.

•Collaborates with other nurses and physicians to identify, promote, and provide positive learning experiences for staff and students.

•Initiates, conducts, and analyzes evidence-based practice research to improve patient care outcomes.

•Serves as preceptor, mentor and resource to other nursing staff and students.

•Strongly encouraged to obtain a board certification related to transplant such as ABTC or CCTC within two years of hire.

•Attend practice related educational opportunities such as conference and workshops\

•Participates in educational and outreach activities

Minimum Qualifications

school diploma or equivalent.

2. Graduation from an accredited school of nursing.

3. Eligibility for registration with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.

4. Two years of transplant experience in nursing (RN)

5. Current AHA BLS/CPR and other required unit specific certifications

6. Bachelor's in Nursing (BSN) required.

7. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers

8. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training Current Illinois nursing license required.

Preferred Qualifications

Transplant or Organ specific certification preferred (must obtain within 2 years of employment)

MSN strongly preferred.
