Shift : MondayFriday, 85, with possible weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract opportunity in Glastonbury, CT. This position involves providing skilled in-home nursing care, completing accurate OASIS assessments, and managing a field caseload. Candidates should be comfortable working independently and delivering high-quality patient support.

Key Responsibilities :

Perform start-of-care assessments, follow-up visits, and discharges

Complete OASIS documentation via Homecare Homebase (HCHB)

Deliver skilled nursing care such as wound care, IV therapy, and medication management

Educate patients and caregivers on treatment plans, disease processes, and safety

Coordinate care with physicians and interdisciplinary team members

Qualifications :

Active Connecticut RN license

Minimum 2 years of nursing experience, including at least 1 year in home health

OASIS and Homecare Homebase (HCHB) proficiency required

Current BLS certification

Reliable transportation, valid drivers license, and auto insurance

Able to pass background check, drug screen, and meet Joint Commission standards
