Responsibilities
What you'll do as an Intake Coordinator :
Coordinates the development of a consistent intake process with current process, policy and procedure :Reviews referral documentation to ensure a complete referral has been received.
Communicates with referral sources, our Business Development Team, and / or utilizes hospital EMRs to obtain any missing referral information.Processes referrals and ensures all patient information entered into the electronic medical record is complete and accurate.Contacts patients to welcome them on service with Interim Healthcare, confirm patient demographics, review insurance information, gather additional medical information if needed.Follows up on referrals that are pending daily and communicates referrals pending to the appropriate area.Answers phone and email inquiries.Collaborates on referrals with appropriate staff including the Home Health Scheduler, Clinical Manager, Director, Managed Care, and / or other members of the intake department.Directly responsible for the adherence to the referral process for all Medicare, Medicaid, Private insurance, and all other referrals.Coordinates the implementation of consistent customer service standards. Customer service standards include standards to :Educate customers about our services and provide accurate and relevant information.
Communicate effectively to all customers in a WOW manner.Responds to patient / client care needs by providing complete and accurate information within scope of the Intake Coordinator or refers the caller to the Clinical Manager or other appropriate resource.Documents calls / communication to / from customers and follow up / outcome of calls in the patient record in the EMR.Provides back up of other positions as needed for the purposes of relief / PTO.Communicates individual needs to the Manager and always maintains confidentiality.Required Skills
