How you can make an impact : As a Licensed Clinical Social Worker you will provide Behavioral Health consultation as part of an interdisciplinary team-based model of care. Our patients receive care in the OBGYN, Pediatrics, Internal Medicine, MAT, and Dental Departments. What you will do :

• Conduct brief / short-term / intermediate therapy for individuals and families
• Conduct biopsychosocial assessments and be familiar with other clinical assessments and use of the DSM in diagnosing and treatment
• Establish and formulate comprehensive treatment goals / plans
• Complete timely and accurate documentation in EMR
• Actively participate in case conferences with behavioral health, social work staff, and other clinical staff
• Make appropriate patient referrals for psych evaluations and medication management services
• Maintain and manage provider schedule and maintain active patient list (caseload)
• Attend monthly departmental meetings and supervision (if applicable)
• Educate patients in the health center and community in the nature of behavioral, mental, and social health determinants on mental health
