Home Health Aide (HHA) / Caregiver
You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a high-impact, patient-facing caregiving role. You'll be a trusted presence in the home helping clients stay safe, comfortable, and independent. Your work directly supports quality of life for patients and peace of mind for their families. Key responsibilities
Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
Prepare meals and assist with feeding as needed
Support medication self-administration through reminders (no independent med administration)
Monitor and report patient condition and changes to the supervising nurse
Take and record basic vital signs when trained and assigned
Maintain a clean, safe, and orderly client environment
Assist with linens and light client laundry as appropriate
Use safety equipment properly (rails, supports, stools, etc.)
Document care provided according to company and regulatory standards
Travel reliably to patient homes and assigned visits across your service area
Complete required in-service training, including annual dementia education

Completion of required certifications and training on schedule
Strong communication with supervising nurse and care team
Must-have requirements
Age 18 or older
Active HHA or CNA certification or willingness to obtain certification
Willingness to complete competency and skills assessments
Ability to follow written care instructions and document services
Compassion, patience, and professionalism with vulnerable populations
Ability to work independently in home settings
Reliable transportation and ability to travel to client homes
Ability to lift up to 25 pounds
Willingness to complete TB testing, background screening, and onboarding requirements
Nice-to-have
6+ months of caregiving or direct patient care experience
Home health or home care experience
CPR and Adult First Aid certification
Experience supporting seniors or medically complex clients
Experience working with dementia or memory care patients
Work environment & schedule
One-on-one care in patient homes and occasional facility settings
Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
Assignments may include homes with pets or smokers
Travel within your preferred coverage area
