About The Position

Certifications : Required certifications applicable to nursing and case management

Must-Have :

• Hospital experience
• Experience with assessing patients
• Experience in care coordination
• Experience with discharge planning and utilization review
• Ability to organize and participate in family meetings
• Knowledge of adoption / foster care, drug / alcohol abuse treatment options
• Competent with arranging durable medical equipment, skilled nursing facilities, and transportation

Description :

The registered nurse will be responsible for managing a caseload of up to 25 patients.

The role requires the nurse to work a minimum of four weekend shifts in a six-week period.

The registered nurse will participate in the development of treatment plans.

Experience with HMOs and IPAs is desired.

Knowledge of patients experiencing homelessness or in crisis is beneficial.

The nurse must be able to perform tasks independently.

Interdisciplinary support will include physical therapy and social services.

Excellent organizational skills are essential for this position.

The nurse will be expected to float between facilities within a given state.

References from supervisory roles covering a total of one year of employment are required.

All travelers are expected to float between facilities, and refusal to do so may result in termination of the assignment.

Providers may need communication devices to perform their duties, and if not provided, clients are responsible for reimbursing work-related cell phone usage.

Modules for ongoing education average 16 hours and must be completed pre-start and annually.

Completion of required modules will be self-reported.
