Position Summary :
The LPN Charge Nurse will be responsible to provide, coordinate and oversee the direct care services for our assisted living residents. This includes following and ensuring that other members of the care team follow the care and services identified in each resident's Individualized Serve Plan. In addition, the LPN Charge Nurse provides direct assistance with medication administration as identified in the Service Plan and ordered by the resident's practitioner. The LPN Charge Nurse is responsible for documenting and communicating resident change in condition and may respond to urgent, or emergent needs of residents in both assisted living and independent living which may include providing care within their scope of practice, including summoning and / or facilitating emergency medical services, as indicated. The LPN charge nurse ensures the completion of all assigned tasks by the other members of the health care team. Key traits include empathy, compassion and the ability to communicate effectively with other team members, practitioners, residents and families. The LPN Charge Nurse may assist with onboarding and training of new team members and will act as a mentor for other members of the care team.
This position reports directly to the Health & Wellness Director, or Executive Director of Assisted Living.

Education : Graduate of LPN nursing program
Experience : 1 year of nursing / prefer experience in Assisted Living
Computer Skills : Ability to effectively utilize basic software and email
