Care Coordinator II

JOB SUMMARY

The Care Coordinator II is responsible for providing individualized community support services to clients referred from the Office of Aging and Adult Services.

The goal is to assist individuals / household to obtain and maintain housing successfully within the community while embodying the performance standards as outlined below

ESSENTIAL DUTIES AND RESPONSIBILITIES :

• Develop, implement, monitor, evaluate and modify a person-centered housing support plan to ensure consumers are receiving services as described in contract
• Ensure compliance with the weekly performance standards, billing 124 units / 31 hours per week for 52 weeks.
• Develops and implements individuals service plans to ensure consumers are receiving services as described in contract.
• Provides transportation to consumers as appropriate.
• Completes reports and documentation as required by Volunteers of America / Greater New Orleans, Inc. and regulatory agencies / funding sources.
• Prepares accurate and timely documentation for billing of services.
• Adheres to the performance standards as identified by the PSH program
• Keeps supervisory staff apprised of gaps in services availability and assist in efforts to provide needed services.
• Services as advocate for consumers receiving services.
• Make referrals to community-base services.
• Assist in accessing and maintaining entitlements / benefits.
• Assist tenants in meeting obligations of tenancy.
• Assist tenants in problem-solving and conflict resolution
• Maintain assigned program records
• Ensures eligibility of services prior to performing services
• Assist other team members as needed
• Participates in program related training as required.
• Performs related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES

This position requires the incumbent to have completed education at the Bachelor’s degree level in social service filed an done year post experience working with individuals who are of the eligible population in a community setting.

Have a desire to work with a diverse population, demonstrate respect for individuals and demonstrate ability to work in a team environment;

requires a demonstrated willingness to be responsive to the needs of the target population. The position requires excellent verbal and written communication skills and the ability to function as part of a team.

PERFORMANCE STANDARD :

• Bill 31 hours per week / 124 Units
• Ensuring all clients billed are eligible for service
• 80% of all units must be direct face to face contact
• Must maintain good standing on student loans (non-default status).
