Auberle Seeks : Foster Care Coordinator of Intake

Auberle's Foster Care Program provides a home setting for children and youth that cannot be with their family. A range of foster care services are offered in order to meet the needs of young people and their families, when possible.Auberle staff work in tandem with placing agencies to transition young people into permanent living arrangement, whatever that may be. The Foster Care Program expects that the Sanctuary Model of Trauma Informed Care is fully supported and integrated into the approach regarding services along with the Therapeutic Foster Care. The Foster Care Program includes Auberle's SWAN service offerings.SWAN services at Auberle currently include, but are not limited to, Child Preparation, Child Profile, Family Profile, Matching & Post-Permanency units of Service.
ESSENTIAL RESPONSIBILITIES :
- Promote the philosophy and mission of Auberle in the provision of all responsibilities.
- Receive phone, written, and posted referrals from referral sources while maintaining a positive, professional, and supportive relationship.
- Act as a liaison to community social service agencies, youths assigned attorneys, OCCYF staff, foster families, young people in care, their families, and additional service providers. Represent Auberle Foster Care via professional and community affiliations as requested.
- Learn and implement trauma-informed approaches to service delivery and agency culture and functioning by working within the Trauma Informed & Therapeutic Foster Care.
- Ensure that referral information is accurate, thorough, received timely, and distributed to the appropriate team members as necessary.
- Complete SWAN training requirements in order to provide necessary units of service to Allegheny and surrounding counties.
- Accept and process referrals effectively and efficiently.
- Document case activity thoroughly and efficiently within established timeframes.
- Function as a team member and maintain comprehensive communication at all times.
- Ensure completion and quality of all paperwork and accountability reports according to established deadlines. Ensure the completion of work schedules and time sheets.
- Communicate relevant financial information to department in a timely manner.
- Ensure follow through with all intake, referral, and discharge activities.
- Coordinate all paperwork requirements relevant to authorization of services.
- Create promotional materials for program & families looking for placement
- Confirm payment arrangements with referral sources.
- Establish and maintain professional relationships with external service providers to :
Determine the appropriateness of referrals and / orplacement resources
Support an efficient and expedient referral and / orplacement processRecommend new or restructured services to better serve the needs of children and families at riskComplete general intake assessment and screening and channel clients to appropriate program, if applicable.Help market the agency to surrounding counties / states.Maintain confidentiality when receiving and exchanging confidential informationProvide direct case coverage within the program as needed.Provide clinical consultation in team, in supervision, and in the field.Assure compliance with SWAN policies and procedures, Sanctuary Certification, State Regulations, County Contracts, and Agency Policies and Procedures.Promote the Continuous Quality Improvement philosophy and maintain all Quality Improvement goals and objectives. Facilitate in goal and objective development.Demonstrate an understanding of and respect for the cultural diversity of consumers, volunteers, staff, and other stakeholders throughout the provision of services.Assure that services to children and families are individualized, strengths and needs based, culturally sensitive, responsive, and goal driven.Perform such other duties asdirected.TYPICAL WORKING CONDITIONS :
Requires working flexible hours, including evenings, weekends, and holidays.Work is performed in an office, in homes, in the community, at events, & remotely.Work is performed in client and resource homes, where conditions may include tobacco- smoke, domestic animals, household pests, mold and other allergens, and other conditions common to domestic environments.Requires travel for appointments, assessments, court hearings, home visits, interviews, events, and meetings.Contact with staff and non-staff (e.g., client families, referents, staff from other community agencies.)REQUIRED SKILLS AND ABILITIES :
Excellent verbal and written communications skill.Demonstrated computer knowledge and willingness to learn and utilize new data entry programs as assigned.Ability to communicate a passion for Auberle's mission of working with children and families.Ability to work independently with excellent time management skills.Demonstrate the commitment to diversity and inclusion, valuing a diversity of perspectives and encouraging contribution by all team members.Ability to network and build consensus inside and outside the agency.Qualifications
