Schedule: Monday–Friday, 8:00 AM–5:00 PM
Patient Volume: 12–15 patients per day
Practice Model: Outpatient only, value-based care, no hospital responsibilities

Ideal Candidate
\n
- \n
- Board Certified/Board Eligible in Family Medicine or Internal Medicine \n
- Experience caring for Medicare Advantage patients \n
- Comfortable managing complex chronic conditions such as diabetes, CHF, COPD, obesity, and cardiovascular disease \n
- Familiarity with ICD-10 coding and STAR quality measures \n
- Experience with eClinicalWorks EMR is a plus \n
