Responsibilities may include :

• Address the mental, behavioral and physical health needs of clients on a weekly / biweekly and / or monthly basis.
• Complete in person nursing assessments as well as medication and chart review in Day Program settings.
• Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
• Care management duties include coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
• Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
• Possibly assisting clinic and residential needs for assigned locations.
• Provide guidance and oversight of nursing services.
• Help train and mentor peers.
• Will cover multiple day program within Northeast, Ohio.

Requirements for this position include :

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• IDD experience is a plus.
• Willingness to travel throughout assigned service area.
• Preferably has experience working in LTC facilities as well as Behavioral Health.
• CPR or Course can be provided during training
