Patient Care Assistant

Under supervision as an entry-level Patient Care Assistant, this role is responsible for providing direct patient care. The Patient Care Assistant participates in data collection, planning and implementation of patient care.

Medical Assistant Co-Op, Care Now

Under supervision as an entry-level Medical Assistant this role is responsible for providing direct patient care in an outpatient setting.

The Medical Assistant’s primary responsibility is to assist in the delivery of health care management for the practice. This will include actively participating and supporting primary clinical teams.

The Medical Assistant is a professional member of the interdisciplinary team and consistently demonstrates and promotes the values of the organization.

The Medical Assistant will work with and receive supervision from the practice manager, physicians, nurse practitioners and physician assistants.

Lab aide

Under supervision as an entry-level lab aide, responsible for registration of laboratory orders, accessioning and transport of specimens, internal and external customer service, and accessioning equipment cleanliness.

Requires independent judgment in the performance of the assigned duties.

JOB DUTIES AND RESPONSIBILITIES : Lab aide

  • Accessioning and transport of specimens to the appropriate testing area, performing campus or reference laboratory.
  • Operates a centrifuge according to procedure.
  • Labels blood and non-blood specimens with barcoded labels and aliquots specimen if necessary, according to policy.
  • Assists with requisition entry task when necessary; works with Lab staff to ensures registration completes the process.
  • Adds cytolyte to cytology specimens according to policy.
  • Learns computer downtime procedures.
  • Prepares specimens for referral (interfaced and non-interfaced tests) to outside reference laboratories which include logging, packaging, and mailing of specimens.
  • Reviews outstanding list, excepted list, and / or add-on list to assure all specimens accessioned and transported to appropriate testing facility.

Job Duties & Responsibilities : Medical Assistant

  • Actively participates in maintaining and / or improving quality improvement initiatives.
  • Effectively manages the scheduling of appointments and tracking of referrals.
  • Assists in capturing patient demographic information.
  • Actively participates in department activities to include : attends departmental meetings; actively participates as a team member in resolution of problems as they are identified;

analyzes current procedures, bringing suggestions for improvement to the attention of team members and supervisors for consideration.

  • Conducts pre-visit planning by reviewing and updating the patient’s medical record prior to the appointment with recent test results and correspondence and determines needed services by conferring with physician, nurse practitioner or physician assistant.
  • Completes pre-charting of all value based pieces up to 5 days prior to patient’s appointment. Send messages to the Care Team, when warranted.
  • Responsible for prepping patients for their examination and escorting them into the exam room. Preparation includes height, weight and vital signs;
  • update medication list; update allergies in record; update personal, family and social history; inquire about smoking status;

obtains the reason for the visit.

JOB DUTIES AND RESPONSIBILITIES : Patient Care Assistant

  • Performs the following tasks as assigned : Vital signs, intake and output measurement, and documentation for all assigned responsibilities.
  • Follows treatment plan as directed by licensed personnel.
  • Performs necessary functions to ensure patient comfort and satisfaction with service.
  • Performs patient mobilization using appropriate and proper body mechanics and assistive device.
  • Accepts responsibility for providing safe, appropriate, quality patient care.
  • Communicates changes in patients’ conditions and unit concerns with the unit based team.
  • Restocks unit supplies as needed.
  • Performs equipment checks and unit specific maintenance duties as appropriate to unit.

SLUHN NETWORK SPECIFIC ESSENTIAL FUNCTIONS

  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates / models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).
  • Demonstrates Performance Improvement in the following areas as appropriate : Clinical Care / Outcomes, Customer / Service Improvement, Operational System / Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.
  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
  • Performs other related duties as assigned or requested.

PHYSICIAL AND SENSORY REQUIREMENTS : (Lab Aide)

Walking for up to six (6) hours in half-hour increments. Frequently uses fingers for typing data entry, and hands for processing specimens which includes twisting and turning specimen caps.

Continuously uses upper extremities for transporting of specimens up to ten (10) pounds. Rarely lifts, pushes or pulls from 26 to 50 pounds.

Rarely stoops, bends or reaches above shoulder level. Hearing as it relates to normal conversation. Seeing as it relates to general vision, near vision, peripheral vision and visual monotony.

PHYSICAL AND SENSORY REQUIREMENTS : (PCA)

Requires sitting, standing and walking for extended periods of time (up to 8 hours at time). Requires continual use of fingers for patient care, writing and computer entry.

Routinely uses upper extremities; occasional requirement to lift up to 25 pounds. Occasionally push cart with supplies up to 30 pounds.

Occasionally push wheelchair with patient weighing up to 325 pounds. Regularly requires ability to stoop, bend and reach above shoulder level.

Requires ability to hear normal conversation and good general, near and peripheral vision.

EDUCATION :

Enrolled in a healthcare based vocational technical school, or similar program
