• Clinical Information :

Covering Emergency Department and Hospital consults and emergencies

• Only ED and Hospital work, no office
• Cover emergencies and any needing surgery
• Patient volume : 0-10 daily
• Required :

CV must be in Month / Year format

• Must have license in hand at time of submission, or proof that the license application has been submitted
• Any gaps greater than 30 days will require an explanation on the CV
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
• Current Flu Vaccine
• Must be fully vaccinated for Covid 19

Exemptions only allowed for religious or medical reasons, once approved by the facility.

• Clean & Active IN License and DEA (or easily transferrable DEA)
• EPIC experience - Preferred
• Board Certified in Urology
• Must be available to work 5 / 5 / 25-5 / 12 / 25
• Required Procedures / Common Diagnosis : Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney, Benign prostatic hyperplasia with lower urinary tract symptoms, Calculus of ureter, Urinary tract infection
