50 annually, plus bonus and equityJob Description : This role involves providing primary healthcare to seniors, allowing them to live in their communities and homes.

By working under the Medical Director's direction, the Nurse Practitioner collaborates with a team of physicians and interdisciplinary teams to offer comprehensive medical services.

This includes assessments, care planning, and continuous medical support to enhance participants' quality of life.Required Qualifications : * Master's degree from an accredited nursing program.

• Certified Nurse Practitioner with Prescriptive Authority.* Valid California NP State License.* A minimum of two years' experience with the senior population.
• In-depth knowledge of chronic care and geriatric best practices.* Clean DMV record and valid state driver's license.* Recent vehicle insurance copy.

Preferred Qualifications : * Prior experience in care coordination and policy improvement is an advantage.Responsibilities : * Conduct comprehensive, initial, semi-annual, and unscheduled assessments of participants.

• Collaborate with physicians and interdisciplinary teams to develop and implement care plans.* Participate in discharge planning and maintain participant care in various settings, including nursing homes.
• Review data on outcomes and utilization to guide best practices and policy enhancements.* Engage in rotating night and weekend calls.
