Schedule : Monday - Friday 8 : 00 A.M. - 5 : 00 P.M. (Some Evenings and Weekends May Be Required)
Salary : $136,000 - $180,000

Job Description
- Ability to Meet Clients Where They Reside( Nursing Home, Hospital, Personal Residence, Virtually Etc.)
- Assesses, Initiates and Coordinates Plan of Care for Patients
- Liases with Other Team Members for Plan of Care Adjustments and Updates Including Behavioral Health and Pharmacy
- Initiates, Evaluates, Reviews and Changes Orders to Medication as Needed
- Makes Referrals to Specialists as Needed
- Preforms Direct Patient Care Including Conducting Physicals
