HealthRIGHT 360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Residential programs run 24 / 7 and Individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, Medication support services, and crisis intervention.

Located on the Be Well campus, a multi-service behavioral health center for Orange County residents, HealthRIGHT 360 provides gender responsive withdrawal management and residential treatment for adult men and women. The program offers a therapeutic milieu to improve the health, safety, and self-sufficiency of people struggling with addiction. Be Well Program is a residential substance use disorder (SUD) treatment program that provides 3.1, 3.3, and 3.5 ASAM levels of care for males and females, as well as 3.2 withdrawal management. There are 15 beds allocated for 3.1 / 3.5, 15 beds for 3.3 and 12 beds for 3.2, for a total of 42 beds.

The Licensed Vocational Nurse (LVN) works in collaboration with other members of the multidisciplinary team to ensure the best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360's philosophy, goals, policies, mission, and vision. LVN assists the Nursing Supervisor, Nurse Practitioners, and Program Director with medication management and education, chart audits, and daily operations support. LVN takes the lead in crisis intervention and client stabilization while maintaining composure.

LVN has an assigned main responsibility and is expected to be cross-trained and cover other departments on as needed basis based on workload and coverage need. LVN is expected to cover weekends as well as evenings, depending on the shift assignments. LVN is cross trained in Medication / Floor duties, Intake, and Clinical duties.

KEY RESPONSIBILITIES

Medication / Floor LVN / LPT assists with keeping track and storing medication for clients at the residential

facility and helps clients with taking their medications during the med line. Monitors and notifies medical

personnel of medication adherence issues or side effects that clients may be experiencing. Helps clients

with making outside medical appointments and support ancillary floor staff as needed. LVN works a block

shift schedule and will need to cover evenings and weekends.

Intake LVN / LPT helps with medical evaluations for new clients coming into the facility and documents

findings. Coordinates with other intake staff and other licensed medical providers to ensure that incoming

clients are medically stable enough to initiate residential treatment. If incoming clients require further

stabilization, they will work with staff to ensure patients are transferred to appropriate withdrawal

management / detox facilities. Intake LVN is expected to work during weekdays, 8am to 5pm, or as

assigned.

Clinic LVN / LPT works closely with the Nurse Practitioners (NP) who are responsible for providing ongoing

mental health services, medication assisted therapy services as well as urgent primary care services for

clients at the facility. Documents vitals as well as rooming clients prior to their assessment with the NP.

Helps with scheduling, communicates medication prescription changes to the Medication / Floor

LVNs / LPTs and coordinates with other LVNs / LPTs to communicate relevant medical updates to the NPs.

Set up and facilitates zoom meetings for interdisciplinary huddles every morning. Clinic LVN is expected

to work during weekdays from 8am to 5pm or as assigned.

Direct Service

  • Assist the nursing supervisor and associate medical director in the day-to-day monitoring,
  • treatment, and maintenance of client medical conditions as qualification and license allows.

  • Intake process : Conduct assessments (head to toe), COVID swab, TB test, medication pass, and
  • medical triaging.

  • Assist with blood draws.
  • Facilitate health, wellness, and medical education for staff and clients.
  • Develop and assess effectiveness of individualized treatment plans and consumer progress.
  • Assist clients in developing independent living skills to promote independence and self sufficiency.
  • Transport clients as needed.
  • Complete random and scheduled client UA tests and documents test results.
  • Assist with checking in visitors and monitoring visits, ensuring clients sign in / out with a valid pass,
  • monitoring clients when they return from off-site passes, and checking all bags upon entering the

    facility.

  • Sign emergency passes when primary counselor is out of the office.
  • Perform periodic facility checks to ensure and maintain the safety and security of the facility.
  • Conduct random and planned room searches primarily in the withdrawal management units.
  • Documentation

  • Conduct chart audits and report findings to Medical Management and Nurses.
  • Ensure that MARs (Medication Administration Records) are complete and accurate.
  • Maintain documentation in compliance with the agency, HIPAA, DMC, and other regulatory
  • standards and requirements.

  • Maintain up-to-date files and timely documentation in the EHR system.
  • Monitor and document for medical quality assurance.
  • Complete all required paperwork, charting, records maintenance in a timely manner when
  • providing direct services.

  • Submit program and contractual reporting and documentation requirement in a timely manner.
  • Administration and Other Duties

  • Act as liaison for removal of biohazardous waste.
  • Take the lead with transportation requests, client appointments, running Pop Sheet reports, and
  • managing OTC box.

  • Interface with multidisciplinary team in treatment planning and service delivery.
  • Establish working relationships with community partners for continuity of care.
  • Responsible for tracking and ordering medical supplies.
  • Serve as care coordinator for clients and completion of all coordinated care documentation.
  • Arrange work schedule in accordance with the agency's needs.
  • QUALIFICATIONS

    Education, Certification, or Licensure

  • Current licensure as a Vocational Nurse in California.
  • Completion of an accredited LVN program.
  • Basic Life Support certification.
  • Valid California driver's license.
  • Experience

  • Experience and commitment to working with diverse communities.
  • Experience serving seriously and persistently mentally ill clients with co-occurring challenges.
  • At least 3 years of LVN experience preferred.
  • One year's work experience in a residential substance abuse treatment setting or a withdrawal
  • management or detox setting desired.

    Must complete background check and livescan clearance.
