Georgia Licensed Care Manager (RN)

A company is looking for a Care Manager (RN).

Key Responsibilities

Develop and assess complex care management activities and personalized care plans for members

Coordinate care between members, families, and providers to ensure access to appropriate services

Monitor member status and revise care plans as necessary to meet individual goals and needs

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

2 - 4 years of related experience in care management

Registered Nurse (RN) state licensure and / or Compact State Licensure required

Experience in behavioral health and / or home health is preferred

Must possess a compact license
