About the Role

As an LPN , you will provide direct patient care under the supervision of a Registered Nurse or physician, ensuring each resident receives the highest quality of care.

Key Responsibilities :

Administer prescribed medications and treatments.

Monitor and record vital signs and patient conditions.

Perform wound care and assist with daily living activities.

Document care in electronic medical records (EMR) accurately and timely.

Communicate effectively with residents, families, and healthcare team members.

Identify and report changes in resident conditions to the supervising nurse.

Follow infection control, safety, and confidentiality protocols.

Collaborate with nurses, physicians, and therapists to ensure coordinated care.

Qualifications :

Current state license as a Licensed Practical Nurse (LPN).

Graduate of an accredited practical nursing program.

Knowledge of nursing principles and practices.

Excellent communication and interpersonal skills.

Ability to work independently and within a team.

Detail-oriented with strong multitasking abilities.

Flexibility to work varied shifts, including weekends and holidays.

EMR experience preferred.
