Job descriptionValid Registered Nurse (RN) license Current CPR certification Basic computer skills Minimum 2 years of experience in Skilled Nursing Charge Nurse, Unit Manager, or Supervisor experience Ensuring the delivery of effective, quality, outcome-focused nursing care Collaborating with residents, responsible parties, and interdisciplinary care teams to develop individualized plans of care Implementing the plan of care consistently and cost-efficiently, with a focus on resident-centered outcomes Utilizing all appropriate resources for resident / family / responsible party counseling and discharge planning Collaborating with physicians and other healthcare professionals to share relevant information for changing the plan of care
