Liaison, Community Integration Professional

This position is eligible for Medical, Dental, Vision, Retirement, Long Term Disability, Short Term Disability, Life and Accidental Death and Dismemberment, Flexible Spending Accounts and several voluntary supplemental benefit offerings. The schedule is Monday - Friday 6 : 30am-3pm.

OBJECTIVE : To assist Day Program staff in effectively implement Service Plans (SPs) and program techniques through community inclusive activities.

QUALIFICATIONS : High School diploma or GED preferred. Medication Administration Qualification (or complete training within 30 days). CPR / First Aid training or willingness to obtain training. A valid Colorado driver's license.

OTHER REQUIREMENTS / PREFERENCES : Previous experience with mild to severe developmentally disabled adolescents or adults is helpful. The incumbent must have good physical stamina / endurance to stand for extended periods of time and demonstrate empathy and concern for individuals with development disabilities.

KEY RESPONSIBILITY AREAS :

• Community Integration : Work with Day Programs team members to schedule community activities coinciding with curriculum.
• Client Care : Assist in the implementation of program plans, giving reinforcers, providing one-on-one assistance and / or training.
• Team Support : Participate in staff meetings, teach proper self-help skills, and provide coverage in programming areas and lunch periods.
• Client / Program Tracking Reporting : Fill out progress reports and assist Case Mangers with completing assessments.
• Bus Care : Responsible for the cleanliness of the vehicle's interior and exterior and ensuring the bus is in good maintenance.
• Relationship Management : Exhibit sound judgement when speaking with and assisting participants and communicate in a timely manner.
