We are seeking Certified Home Health Aide to fill job position in Seaford, Alberston in Nassau county and Brentwood in Suffolk County to become of our team! You will provide routine companion care to Hospice patients at the patient's home or in a care facility, also dementia and greriatric patients in their homes.

Responsibilities :

• Provide routine individualized care to the elderly, convalescents, or persons with disabilities
• Monitor and report changes in patient health status
• Provide personal care
• Assist family members to cope with the clients situation

Qualifications :

• Experience in hospice, home health, or other related fields
• Positive and patient demeanor
• Ability to build rapport with patients
• Ability to handle emotional workload
• Excellent written and verbal communication skills
