Phoenix Physical Therapy & Wellness is growing! We are seeking a Physical Therapist Part Time / PRN to join our IN-HOME OUTPATIENT PHYSICAL THERAPY team! You will have the opportunity to be a part of a therapist owned and operated, fast growing outpatient clinic that is excited about serving the community in which we live!
The Physical Therapist will adhere to the principles consistent with the American Physical Therapy Association (APTA) Code of Ethics and Standards of Practice.
The Physical Therapist assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

Respects and understands the patient’s right to treatment and right to involvement in treatment decisions
Practices ethical responsibility based on the cultural, ethnic and religious beliefs of the patients served and applicable law
Treats patients of all ages (e.g. infants, children, adolescents, adults, geriatrics, elderly, ages 0-120 years), race, creed, gender, and disability without bias or prejudice
Attends regular clinic meetings to report on and discuss activities and needs of the clinic, discuss patient’s needs and progress, discuss reports and records, discuss expansion in terms of future goals and needs
Sees that all clinic routines and procedures are carried out as outlined in Company Policies and Procedures, as well as Employee Handbook.
Keeps abreast of new techniques and trends in the field of Physical Therapy
Represents Phoenix and the location in which clinical services are provided, in a professional and courteous manner, maintaining good public relations with the community we serve.

Essential Duties and Responsibilities :
Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Completes and submits all documentation within the outlined timeline
Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Records prognosis, treatment, response, and progress in patient's chart or enters information into computer.
Identifies and documents goals, anticipated progress, and plans for reevaluation.
Administers manual exercises, massage, or traction to help relieve pain, increase patient strength, or decrease or prevent deformity or crippling.
Evaluates effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Tests and measures patient's strength, motor development and function, sensory perception, functional capacity, or respiratory or circulatory efficiency and record data.
Instructs patient and family in treatment procedures to be continued at home.
Confers with the patient, medical practitioners, or other appropriate parties to plan, implement, or assess the intervention program.
Reviews physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.

Supervises clinic activities as needed, including but not limited to scheduling patients for treatment and evaluations, preparing daily notes and report, maintaining inventory of materials and supplies for their area of the clinic, volunteers who may assist in their area, student physical therapist / Physical Therapist Assistant
Prepares and submits clinic reports, assuring confidentiality and following all clinic policies and procedures to ensure adherence to HIPAA guidelines to include initial evaluations and summaries, daily treatment notes for all patients seen that day under his / her care, progress reports to referring physicians and agencies, discharge reports and summaries, special reports as directed by Clinic Director and daily attendance records and summaries

Required Education, Certifications and Licenses :
Graduate of a CAPTE accredited school of Physical Therapy with a Bachelor’s of Science, Master’s, or DPT.
Experience : Previous experience in outpatient setting preferred
State licensure or be eligible for state licensure
