At Houston Methodist the Pharmacy ClinicalSpecialist position is responsible for performing advanced clinicalpharmacy duties enabling the delivery of optimal pharmacotherapy topatients and facilitating safe efficacious and pharmacoeconomicallyrational use of medications in the population(s) served.

Thisposition involves daily reactive and targeted proactive medicationregimen reviews for patients within the designated area(s) ofresponsibility or populations served.

The Pharmacy ClinicalSpecialist position supports the delivery of pharmacy managedprotocols formulary processes and medication reconciliationactivities while providing education to fellow pharmacists nursesand medical staff.

Additional duties for this position includeprecepting pharmacy students and pharmacy residents on clinicalpatient care rotations within areas of responsibility andcontributing to fulfilling the organizations mission to provideinnovative personalized costeffective pharmaceutical care in aculture dedicated to quality andsafety.

PEOPLE ESSENTIALFUNCTIONS

Promotes apositive work environment and contributes to a dynamic team focusedwork unit that actively helps one another to achieve optimaldepartment results.

Collaborates with other health careprofessionals as needed for problem resolution and / or achievementof identified patient specificgoals

Serves as a rotationpreceptor for students from affiliated colleges of pharmacy andPGY1 or PGY2 pharmacy residents as well as being a mentor for staffpharmacists supporting clinical skilladvancement

SERVICEESSENTIALFUNCTIONS

Processes allorders / requests accurately and efficiently verifyingappropriateness of all drugs in reference to the patients diagnosisallergies height and weight.

Exercises good judgment in patientcare decisions per performance standard and documents patient careactivities and patient outcomes impacted by pharmaceutical care andclinicalinterventions

  • Providespatientspecific and nonpatient specific drug information supportregularly
  • Serves as amember on one or more Medical Staff Committees and activelycontributes to accomplishing the goals of thecommittee

QUALITY / SAFETYESSENTIALFUNCTIONS

  • Collaborates withother health care professionals to manage patients drug therapy formaximal effectiveness while minimizing side effects adverse drugreactions toxicities drug interactions and incompatibilities usingestablished internal and externalguidelines
  • Providespharmaceutical care at the fundamental to advanced level forpatients within the designated practice area(s). Activities includebut are not limited to proactive drug therapy reviews andmonitoring patient counseling transitions of care managementincluding medication acquisition process support and management ofpharmacistbased dosingprotocols
  • Participates inquality management initiatives such as but not limited to : medication use evaluations adverse drug reaction and medicationerror reporting clinical interventions medication center reviewsand inventory management documentation andreporting

FINANCE ESSENTIALFUNCTIONS

  • Contributes to theprovision of pharmacoeconimically sound medication therapy byupholding HM Formulary standards in area(s) of responsibility andavoiding medicationrelatedcomplications
  • Selfmotivatedto independently manage time; effectively prioritizing tasks forself and for others within the specialty area offocus

GROWTH / INNOVATIONESSENTIAL FUNCTIONS

  • Presentsinservices for pharmacy medical nursing and other staff members inarea(s) of responsibility. Develops educational materials andprovides educational programs for the pharmacy staff and otherhospital personnel
  • Seeksopportunities to expand ones learning beyond fundamentalcompetencies with a focus on continual development as a ClinicalSpecialist
  • Contributes tothe design analysis and reporting of quality improvement programfindings
  • Participates inresearch projects to advance organizational quality of care and thepharmacy profession. Submits research and scholarly work forpublication platform presentations and / or posterpresentations

WORKEXPERIENCE

Musthave one of the following :

Successfulcompletion of an ASHPaccredited postgraduate year one (PGY1)residency training program or equivalent experience (i.

e. threeyears of pharmacy experience per residency year); ASHPaccreditedpostgraduate year two (PGY2) or fellowship preferred

  • Three years of clinical practiceexperience
  • Meets HMstandards of an ASHPQualified Residency Preceptor within one yearof employment

LICENSESAND CERTIFICATIONSREQUIRED

  • PHRM PharmacistState Licensure Texas Pharmacist License or eligible for TexasPharmacist License within 90 days and
  • BLS BasicLife Support (AHA) within 90 days and
  • ACLSAdvanced Cardiac Life Support (AHA) within 90 daysand
  • PALS Pediatric Advanced Life Support (AHA)within 90 days and
  • NRP Neonatal Resuscitationwithin 90 days and
  • PPC Pharmacist PreceptorCert State Licensure within 90 days and
  • SCATSterile Compounding and Aseptic Technique within 90 daysand
  • Houston Methodist Hospital ResearchInstitute Certification (see manager for details and trackinginformation) within 90 days
