General Description :
As a Geriatrician at Evara Health, you will play a crucial role in providing comprehensive medical care to the senior population.
You will be instrumental in diagnosing, treating, and managing medical conditions, focusing on maintaining patient well-being and independent functioning.
Functions in accordance with plans and policies established by the Board of Directors and performs duties within the medical setting as a member of the care team providing medical care.
Essential Duties & Responsibilities
- Maintenance of patient well-being and independent functioning
- Diagnoses and treats illnesses and injuries that commonly occur with the aging process
- Closely monitors their patients’ drug regimens
- Advances care planning with patients and families, including discussions about long-term care alternatives, hospice care, etc.
- Provides general care for patients in a patient centered model, commensurate with training, experience, and clinic needs;
to include well check visits, acute illness visits, family planning, etc.
- Maintains timely and accurate electronic medical record documentation on all patients including office visits, telephone calls, hospital visits and admissions
- Prepares timely and accurate billing charges
- Makes appropriate referrals to other health facilities for further testing, diagnosis, treatment and follow-up care
- Manages patient utilization of services
- Co-chairs the activities of the care team daily huddles
- Participates in AAAHC on-going readiness
- Participates in Peer Review and Quality Improvement Program
- Demonstrates commitment to professional growth and competency by maintaining current, up-to-date knowledge in the field of medicine
- Strives to achieve goals of the health care plan and participates in annual updates
- Participates in call schedule as assigned from time to time by medical director
- Timely and accurately inputs patient data into an electronic health record per agency standards
- Maintains current Board Certification in Specialty
- Periodically supervises and teaches Residents, Medical Students, and other allied Health Professionals
- May be required to work multiple locations

Minimum Qualifications :
- Proficient in diagnosing, treating and managing medical conditions in the Senior population with multiple comorbidities
- Trained to evaluate and manage the unique healthcare needs and treatment preferences of older people
- Able to develop and maintain positive working relationships with patients, co-workers, other medical provider, and vendors
- Able to relate with people of different ethnic origins, economic status, religious affiliations or practices, and gender orientation
- Able to operate and communicate clearly via a telephone
- Ability to navigate and enter data into an electronic health record
- Valid and current Florida medical license
- Attendance and graduation from an accredited residency program
- Attendance and graduation from an ACGME accredited residency program in internal or family medicine
- Attendance and graduation from an ACGME accredited fellowship in Geriatrics or equivalent work experience
- Board certified or board eligible in primary specialty
- DEA certificate
