Nurse Practitioner - Geriatrics

Growing practice with locations in the Central Valley is adding a Nurse Practitioner to staff. This position will be based out of Manteca, CA and the ideal candidate will function autonomously, as well as in collaboration with all members of the existing Integrated Care team, to facilitate high quality, cost-effective care to dialysis patients. Nurse Practitioner will be expected to provide coordinated care services between the patient, family, physicians, additional providers, and other care team members. Experience as a Family Nurse Practitioner, Adult Nurse Practitioner, Hospitalist or experience in Nephrology, internal medicine, cardiology, or Case Manager is required.

Candidate must demonstrate proficiency in : clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients. Experience with dialysis patients, cardiac and diabetes education is a plus.

No weekends, no call required. Applicants must have reliable transportation and be able to make house calls when necessary.

Active California license highly preferred.
