Job Description

The Patient Services Representative is responsible for providing high-quality customer service to patients, co-workers, and all visitors to CHC. Pre and post-visit planning, patient outreach, panel management, coordination and facilitation of home visits, efficiently managing clinical patient flow including patient registration, coordinating with clinical staff, answering patient questions, and / or referring patients to other CHC services or departments. Full-time positions available at the following locations : Riverside (2) and Winooski (1) .

A ) Education & Experience :

• High School Diploma or GED
• Prior experience in a related field preferred
