The Qualified Health Home Specialist serves as a coach whose main focus is to support and assist clients in reaching their goals. They interact with the clients on the phone or in person on a regular basis to build trusting relationships. They can meet with client before appointments help clients organize health concerns and prioritize issues to discuss with health providers. They can also meet with client after office visits to review provider instructions and check for client understanding and access to resources to follow provider instructions. They provide ongoing motivation, encouragement, and positive feedback when client makes constructive changes or progress. With the guidance of their supervisor and in collaboration with the BHH team, the Qualified Health Home Specialist is the primary entity responsible for the Health Promotion Services and assists the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination Services.
Job Duties
- Attend and participate in new hire training.
- Attend and participate in 5 week follow up trainings from 8-10am on Fridays after first week of new hire training.
- Builds and maintains a full-time shared caseload up to 56 clients and following BHH guidelines for appropriate service delivery.
- Maintains legible and organized client electronic health records in accordance with policy of NAL and requirements of the State of Minnesota. Concurrent documentation is mandatory.
- Attend regularly scheduled mandatory staff meetings
- Attend regularly scheduled mandatory supervision meetings with assigned supervisor.
- Assists the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination services.
- Responsible for providing Health Promotion Services encouraging and supporting healthy living concepts to motivate clients and / or their caregivers to adopt healthy behaviors and promote better management of their health and wellness.
- Assuring that the Health Promotion Services place a strong emphasis on skills development through health education and wellness interventions that assist clients and / or their caregivers with monitoring and managing their chronic health conditions to improve health outcomes.
- Responsible for communicating information and updates for the patient registry to the Integration Specialist.
- Understands the expectations for how to use and maintain the patient registry.
- Ensures and reviews that the client receives information about the purpose and the services of the BHH and the person's rights and responsibilities.
- Assists as needed with conducting the initial needs assessment and developing the preliminary action plan.
- Responsible for assisting the client in setting up and preparing for appointments, accompanying the client to appointments as appropriate, and following up with the client.
- Responsible for providing coaching to clients and their identified supports.
- Responsible for ensuring clients are aware of resources and are supported in efforts to access resources in order to address the client's identified goals and needs (i.e., county social services, housing, employment).
- Responsible for developing and nurturing relationships with other community and social support providers to aid in effective referrals and timely access to services
- Providing Health Promotion and Wellness activities from a holistic approach and should assist the client and / or client's caregivers in gaining relevant knowledge and skills which will :
- Increase their understanding of the illnesses / health conditions identified in the assessment and how these conditions relate and impact various areas of their life.
- Increase their knowledge of illness-specific management as well as overall, daily health maintenance.
- Support them in activities aimed at increasing their self-efficacy and reaching their health goals.
- Support them in recovery and resiliency.
- Assist client and / or client's caregivers to make healthy lifestyle choices within their budget.
- Support client and / or client's caregivers with improving their social networks.
- Provide wellness and health-promoting lifestyle interventions such as, but not limited to, substance use prevention / early intervention and harm reduction, HIV / AIDS prevention / early intervention, STD prevention / early intervention, family planning and pregnancy, smoking prevention and cessation, nutritional counseling, obesity reduction and prevention, increasing physical activity, and promoting independence and skill development related to self-administration of medications.
- Works collaboratively with community-based agencies in the provision of BHH.
- Supports and participates in the marketing and promotion of the program as indicated.
- Has an understanding of and refers to NAL programs and coordinates care.
- Follows policies and procedures of Nystrom & Associates (NAL).
