Required Qualifications Occupational Therapist, Rehab Experience : 1 year Licenses : OT-CO Certifications : BLS(Copy Needed) SSN Required DOB Required References : 1 Reference in entire work history -Bachelor's degree in Occupational Therapy from an accredited school is required.
