Unit Description – Case Management Department
Type of Care :
Provides comprehensive case management for acute care patients.Focuses on care coordination, discharge planning, and resource utilization.Ensures safe and efficient transitions of care across the continuum.Patient Population :
Adults and geriatrics with acute and chronic medical conditions.Supports patients with psychosocial needs, mental health history, substance use, homelessness, and complex discharge barriers.Average Caseload :
Up to 25 patients per Case Manager.Shift & Schedule :
8-hour shifts (08 : 00 AM – 04 : 30 PM).Flexible scheduling to meet departmental needs.Minimum of 4 weekend shifts per 6-week schedule.Weekend rotation and float coverage required.Call coverage as assigned.Staffing Ratios :
Days : 1 : 25No LVN, Tech, or CNA support assigned to Case Management.Core Responsibilities :
Perform patient assessments and coordinate individualized care plans.Lead and participate in family and interdisciplinary meetings.Manage discharge planning and continuum of care arrangements.Coordinate DME, SNF, rehab, and ambulance transport independently.Collaborate with HMOs, IPAs, and insurance providers.Provide education to patients and families on discharge processes and community resources.Monitor, evaluate, and document patient progress and outcomes.Knowledge of advance directives, POLST, and protective service processes.Documentation & Systems :

Pharmacy, Radiology, Respiratory Therapy, Physical Therapy, Social Services, and Transportation.24 / 7 in-house Hospitalist coverage.Access to interpretation services, lift teams, and rapid response teams.Special Notes :
Hospital experience required — behavioral health experience does not qualify.Must have acute care case management and discharge planning experience.Extra checklist required before submission.Case Managers must demonstrate independence and efficiency in a high-volume setting.Certifications :
BLS – RequiredExperience Requirements :
Minimum 2 years of acute care case management / discharge planning experience.Must be a seasoned traveler (first-time travelers not accepted).Orientation & Additional Info :
