Key Responsibilities

Provide support to the Care Management team in coordinating patient care

Perform complex tasks related to team functions and activities during patient assessments and management

Collaborate closely with healthcare team members to ensure effective care delivery

Required Qualifications

5 years of experience in a hospital, clinical setting, or healthcare-related field

3 years of demonstrated outstanding customer service experience

3 years of clerical and computer experience

3 years of knowledge in insurance billing and managed care

High School Diploma or GED equivalent; some college coursework preferred
