POSITION SUMMARY
The Social Workershall be responsible providing case coordination and full range of social services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. Facilitates Team meetings and physician staffing. Resolves customer service issues and maintains high customer service satisfaction. The incumbent shall demonstrate accountability of contribution to program development, quality improvement, and problem solving and productivity enhancement in a flexible interdisciplinary fashion. The incumbent shall coordinate department customer service / patient satisfaction activities and data collection as well as after care activities including follow up calls and follow up data collection. Contributes to the success of the department through effective work coordination, management of resources and participation in goal accomplishments. Supports and communicates SGMC mission, vision, values, goals and direction to assigned team members. Manages and monitors resource utilization. Performs other duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES
Graduate of a Master's degree CSWE program with LMSW license to practice in state of Georgia.
ACSW certification is desirable.
Experience in working with adult and geriatric population with physical disabilities is desired.
Ability to work well individually, as a team member, and as a team leader.
Demonstrated ability to adapt to new situations, manage priorities and utilize problem-solving techniques.
Ability to read and interpret policies, standards, and think critically evaluating patient care issues.
Strong skills in assessment, planning, intervention and evaluation.
Ability to communicate clearly with, and care for a diverse population of patients / family units of all ages and cultures, and / or persons occasionally exhibiting stressed behaviors.
CPR Certification required.
QI credentialing is required within initial 90 day period, or within 90 days of orientation to Inpatient Rehab unit.
On-site training provided for full time and part time IRF employees.
