HEALTH EDUCATOR SCI SENIOR ABUSE
The Health Educator serves as a liaison and provides guidance and social service assistance to Geriatric participants. The system health educator advocates for individuals by providing referrals and follow-up services through care coordination. This includes face to face meetings with clients to help complete application for Medicare, Medicaid, PACE, LiHeap and other senior services.
MINIMUM Education :
- Bachelor’s degree in Education preferred.
MINIMUM WORK EXPERIENCE :
