Core Responsibilities :

As an LPN, you are central to our patient care team, ensuring residents receive attentive and accurate treatment.

Daily Documentation : Accurately record a patient’s medical history, daily observations, and responses to treatment in the electronic health record (EHR).

Physician Order Management : Precisely transcribe physicians’ orders to Medication Administration Records (MARs) and promptly notify the pharmacy of new orders or changes.

Medication & Treatment : Prepare and administer medications as ordered by the physician, including injections, prescribed medications, enemas, and perform other treatments within your scope of practice.

Monitoring : Take and record measurements of vital signs (blood pressure, temperature, heart rate, etc.), and observe patients under treatment to identify progress, side-effects of medications, and monitor fluid intake and output.

Charting : Accurately compose and maintain patient charts, ensuring complete and descriptive narrative notes.

Resident Care : Assist ailing patients in daily necessary activities such as eating, bathing, dressing, and mobility.

Wound Care & Basic Medical : Clean and dress wounds and assist with other basic medical care tasks as needed.

Sample Collection : Collect samples for lab testing as assigned by the physician or RN.

Support & Communication : Provide emotional and psychological support when needed, and communicate effectively with patients’ family or friends to provide advice, comfort, and release instructions.

Perform all other duties as assigned by the Director of Nursing or Charge Nurse.

Qualifications :

Active and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin (required).

Compassionate and patient, with the ability to remain calm and make sound decisions in a demanding environment.

Experienced in providing diverse basic care to those who need it.
