- Supported Living Caregiver Reports To : Team Lead Position : Part-time, Hourly Location : Kenosha Salary Range : Based on Experience Overview The Crabtree Supported Living Caregiver provides essential, compassionate, and personalized care to residents within an apartment setting.
- This role supports the daily physical, emotional, and social well-being of residents, ensuring they maintain the highest possible quality of life in a safe and supportive environment.
Key Responsibilities Health Monitoring and Documentation Observation :
Monitoring and reporting any changes in a resident's physical, mental, or emotional condition to the Team Lead.Medication Management :
Providing timely and thorough medication monitoring support.Doctor Appointments :
Attending scheduled appointments and providing Team Lead with updates.Documentation :
Completing accurate and timely records of all care provided, observations made, and incident reports using the designated electronic or paper system.Apartment and Personal Care Support The Caregiver is responsible for providing guidance, support, encouragement and prompting for daily living skills.Tidiness :
Ensuring the resident's apartment is safe, tidy, and organized.Laundry :
Ensuring the resident’s laundry is maintained.Nutrition :
Ensuring the resident has access to nutritious food and is eating regular meals. Helping with meal planning and grocery shopping.Social and Emotional Support Engagement :
Encouraging and assisting residents to participate in scheduled social activities and programs.De-escalation :
Providing active listening to resident concerns and addressing day to day needs (i.e. minor roommate concerns and shopping requests).Transportation :
Assisting with transportation as needed including, grocery shopping, doctor appointments, work transportation and bus training.Communication Attend and participate in quarterly meetings and as requested.Regular communication and collaboration with other caregivers, Team Lead and Team Supervisor.Comply with all Electronic Visit Verification (EVV) steps.Collaborate with members of the client's care team, under the guidance of Team Leads.Reading and completing house logs, resident documentation, incident reports and schedules.Carry and answer a company cell phone during shift.Report any issues or concerns immediately to the Team Lead or Office Coordinator.Consult the on-call supervision schedule and contact the designated individual for the shift.Other Duties Complete all scheduled daily tasks, seeking clarification from Team Leads and Office Coordinator as needed.Ensure all company vehicles are maintained, clean and fueled.Other duties as assigned by Team Lead or Team Supervisor.Qualifications Must have and maintain a valid driver’s license in good standing.Must have a minimum of a high school diploma or equivalent.Must be able to give and receive feedback in a professional manner.Must be proficient in reading, writing and speaking English.CBRF training preferred.Working Conditions and Physical Demands Must be able to lift at least 20lbs.Must be able to sit for extended periods in an office environment or in a vehicle.Must be able to traverse in home, office and community settings.Equal Opportunity Employer We are committed to providing equal employment opportunities to all qualified individuals without regard to race, color, religion, gender, sexual orientation, age, national origin, disability, or veteran status.
