RiseWell Community Services is looking for a full-time Master Level Counselor – Family Specialist to join our Forensic Assertive Community Treatment Program in Queens, New York.

In this role, you’ll be utilizing your own professional experience to provide tailored care and guidance to participants and families. The world does not run out of people to help…and we need people like YOU to help us make a positive change in the community!

QUALIFICATIONS :

Master’s degree or higher in social work, psychology, mental health counseling, or marriage and family therapy. Rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred). A valid and clean NY State driver license. Minimum 1 years of experience in a treatment or rehabilitation setting. Fingerprinting, criminal record check. Approval from NYS Office of Mental Health

CORE & ESSENTIAL RESPONSIBILITY :

Provide direct services with a focus on family services Work on a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness. Maintain case records for caseload assigned by the Team Leader. Schedule regular meetings with participants. Conduct group sessions as per staff discipline Collaborate with families and identify needs, strengths, goals, and resources to support families’ health and wellness. Establish, follow up, and maintain new referrals. Complete Needs Assessments. Complete weekly progress updates. Complete visits based on requirements from Team Leader. Prepare and submit reports by deadlines
