SUMMARY
Family Service League is seeking a Full-Time Case Manager to provide persons over 60 years of age with comprehensive coordination of health services, financial assistance, and referrals through the LI Sound Senior Care Program in Greenport.

QUALIFICATIONS :
Associates Degree required.
Bachelor's Degree in Human Services, Social Work, Sociology or related field required.
At least five years' experience assessing and linking adults to needed services and programs required.
Excellent interpersonal and verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required.
Bilingual in Spanish preferred.
PHYSICAL REQUIREMENTS
This position often requires sitting for extended periods of time and traveling to conduct home visits.
