About The Position

Job Description & Requirements

Physical Therapist - Home Health - (PT - HH)

StartDate : ASAP

Available Shifts : 10 D

Pay Rate : $1534.08 - $1680.96

Tell us about the unit(s) :

• Number of Beds – approximately 70 patients that are seen by any combination of disciplines
• Number of Staff – 25 (including nursing, rehab, office, and administrative staff)
• Type of staff : Works with other PT’s, PTA’s, OT, RN’s, CNA’s, SW. This service is carried out where the patient calls home : personal home, ALF

o Day Shift – Office open 8-4 : 30 Monday-Friday (Nurse on call 24 / 7)

o Night Shift - NA

Patient Ratios – 6 visit equivalents per day ( types of visits are weighted differently on a point system)

EMR - Cerner

Skill Set Most recruited for : Home Health

Must have : candidates without these skills will not be considered for the role.

Experience in completing OASIS Assessments

Home Health experience

Preferred or nice to have : candidates with these skills will be considered first.

Some knowledge of new payment model coming 1-1-20 (PDGM)

List typical procedures performed on unit(s) :

Comprehensive assessments / evaluations, treatment and care planning. Oversight of PTA’s.

Patient population : mainly geriatric; post-acute treatment of THA / TKA patients; treatment of patients with chronic illnesses and multiple comorbidities.

Best personality Fit :

Someone who is easy going, likes to meet people and doesn’t mind visiting people in their own homes. Needs to be accepting of others and friendly.

REQUIRED CERTIFICATIONS :

BLS / CPR certification. Needs current NC license to practice Physical Therapy

Dress Code :

Not required to wear scrubs. Business casual, closed toed shoes.

Unit Guidelines / Policies : Please feel free to attach unit policies and guidelines.

Will be discussed in interview.

Weekends and On call(if any)

Weekends only as needed, not as a routine expectation
