MSW - Discharge Planning Experience required Start date : ASAP Shift : 5x8HR Days (0700-1530) Length : 13 weeks Years of experience REQ : 1+ year First-timers accepted : No, not at this time Weekend REQ : Possible weekend shift required, 4 per contract Certs REQ : BLS, MSW Will this traveler need to float between like units : yes, possibly in low census situations. Locals accepted : NO - Local travelers with permanent residence within 50 miles of the facility will not be eligible for a travel contract. Pending License accepted : No, WA license must be in hand at submission RTO Restrictions : Please note all RTO at submission Required Case management skills : Admission Criteria, Care coordination, Discharge Planning, Utilize InterQual Criteria and Utilize Milliman Guidelines Document System : EPIC and AIDA Average Daily Census : 385 Manager / Unit : Shannon Burke / Care Management (MSW),1500.875100.13137 Hospital Highlights : Providence St. Peter Hospital Type of Facility : Short Term Acute Care Total Staffed Beds : 325 Charting : EPIC Floating is required.
